CDTL

Case - 1 : A young patient presented with sudden painful swelling of left lower limb. Doppler showed extensive deep venous thrombosis involving all the deep veins of left lower limb and left lilac veins also . Since it was fresh thrombus , catheter directed thrombolysis was offered to him.

Through the left popliteal vein , venogram was performed which shows extensive large irregular filling defects suggestive of thrombosis.

Extremely poor filling of femoral veins suggests large load of thrombus.

Extremely poor filling of iliac eins suggests large load of thrombus which is  seen as filling defects

Guide Wire was manipulated through the guiding  catheter  to fragment the thrombus.

 Thrombolytic  medicines were infused for 48 hours through the catheter in the thrombus. Intermittently periodic check venogram was performed to check the effect of drugs. 

 

After 46 hours , venogram shows that most of the thrombus is resolved (white arrow) . Black arrow shows residual thrombus.

After 46 hours, significant thrombus resolution is seen in iliac veins also.However residual thrombus is  seen as stenosis

Follow up venogram after 2 weeks shows restoration of left iliac lumen to normal.
