Extracranial carotid aneurysm treatment

3 Year old child had tracheostomy as his trachea was compressed due to vascular mass. It was diagnosed as giant aneurysm of internal carotid artery in the neck.

Contrast was stagnating in the aneurysm due to its large size.

Left Internal carotid angiogram shows giant size aneurysm with narrow mouth(neck).

There was no cross circulation through circle of willis.Hence left internal acrotid artery could not be permanently occluded . It had to be preserved.

Covered stent graft has been used to seal the mouth of aneurysm so that blood does not go in it.

Post stenting angiogram shows excellent restoration of the calibre of internal carotid artery and aneurysm is not filling at all.

Post stenting intracranial circulation is seen normally.

Follow up CT scan after treatment shows stent in situ(red arrow) . Aneurysm is not filling . Yellow arrow shows  calcification of aneurysm wall.

Intracranial circulation  is seen normally after the treatment.
